On July 10, 2006 a man driving a Buick with his wife in the passenger seat through the Interstate 90 connector tunnel in Boston experienced a nightmare when 26 tons of concrete panels fell from the ceiling of the tunnel and crushed his wife, the passenger in the vehicle. The panels simply became detached and fell on the vehicle. The passenger died while the driver had only minor injuries.

It appears that the problem was caused a defective product, specifically by the use of an epoxy anchor. The company that supplied the epoxy was indicted on one count of involuntary manslaughter yesterday. The Attorney General, Martha Coakley, charged Powers Fasteners, the manufacturer of the epoxy, in the death of the victim.
